What's the name of this novel?

When I was 14 I read a book set in the future and the main character, Joseph, goes to an academy, but his sponsor pulls out and he gets kicked out of the academy. Following this, he's taken to the city for a meeting with someone but he gets mugged, ends up with a gang for a short while before eventually meeting a doctor who looks after him near the British coast. On the cover, there's a picture of him running from guards through a futuristic city. Really can't remember what it's called but it may have "home" in the title and was by a quite well known young adult's author
